This project checked the Department's own current Restaurants and Food program page, its Forms & Regulations index, and its own current SF-5 permit application PDF (whose 'Permit Fee' field is left blank for internal office completion) and SF-35 plan-review application PDF, and none of them states a dollar figure anywhere. This is consistent with this project's own West Virginia state-layer finding that the old uniform statewide restaurant permit-fee rule was repealed and county health departments now set their own figures independently — it does not follow that Cabell County's own figure matches the Kanawha-Charleston Health Department's own published figures, and this project found no evidence that it does, so no fee is asserted here.

No fee applies. No fee, because there is no separate sales tax permit to apply for at all — the Business Registration Certificate every West Virginia business already needs is the same document that covers Consumers Sales and Service Tax registration. A business filing the one application and paying the one $30 fee has satisfied both requirements at once; there is no second form, no second fee and no second agency to register with for sales tax specifically.
