About each permit
Health Permit
Four risk levels, and the level sets the inspection frequency as well as the fee. Risk 1 is $275 and one inspection a year, for prepackaged non-TCS food. Risk 2 is $350 and two inspections, for a limited menu with no complex processes. Risk 3 is $400 and THREE inspections, for an extensive menu or complex and specialised processes. Risk 4 is also $400 but FOUR inspections, for premises serving highly susceptible populations — so a nursing home kitchen and a busy restaurant pay the same and are visited at different rates. The November 2025 sheet gives the top band as $400. Lewisville ISD and all 501(c)(3) organisations are exempt from food inspection fees entirely.
Pool and Spa Health Permit
$200 per BODY OF WATER, as republished by the Health Services Division on 15 November 2025; the codified section 2-201 still shows $150 per pool and has not caught up. Per body of water rather than per site means a hotel with a pool and a separate spa pays $400. Note this is the annual health permit to operate; building the pool is a separate $300 construction permit under the Building Inspection Division's schedule, MEP included.
Backflow Testing and Tester Registration
$25 for each backflow device tested and $100 a year for a backflow tester to register with the city. The $25 is corroborated outside the code: Lewisville's own guidance for backflow test submittals states the same $25 filing fee per test, submitted through BSI Online. The device charge recurs annually and is easy to overlook on a site with several assemblies — a restaurant with a kitchen, an irrigation system and a boiler feed can hold three. Your tester must be registered; asking to see it is quicker than a failed filing.
Waste Hauler Permit
$200 per truck. Like Rowlett's liquid waste hauler permit and Wichita Falls' vacuum truck permit, this falls on the contractor rather than on the premises — but it is the premises that suffers if the hauler is not permitted, because the collection cannot lawfully happen. Lewisville runs an online trip ticket system recording the generation, transport and disposal of all regulated liquid waste, with tickets due within ten days of disposal, so the paperwork trail reaches back to your grease trap. The regulations sit at Chapter 16, Article II, Division 4 of the city code.
Health Services Plan Review
$300, confirmed unchanged by the November 2025 schedule. Dearer than Allen's $200 and Irving's $125, cheaper than Plano's $475 for new construction. Charged once rather than banded, so a small fit-out and a large one pay the same.
Temporary Food Service Permit
$75 for the base permit and $10 for each day, capped at ten consecutive days — so a full ten-day run is $175. Note the ceiling is shorter than the fourteen days most Texas cities allow, which matters for a fortnight-long event: you would need a second permit or a different arrangement. Health permit reinspection is $75.
Health Permit Reinspection
$75 per reinspection — mid-range for the Metroplex, below Richardson's $125 and Garland's $200, above Rowlett's $50. A Risk 3 establishment already receives three routine inspections a year, so a failure adds a fourth visit at this price rather than replacing one.
