Two official sources disagree by a wide margin on the alteration-permit base fee, and the department's own PDF is 14 years old (2012) — both figures are disclosed rather than one asserted as current. $500 to form, and Massachusetts is one of the few states where filing online costs more rather than less — a $20 expedite charge applies to electronic and fax filings alike.
