Not settled: whether Mesa separately charges a municipal privilege-license fee under the state's up-to-$50 ceiling (A.R.S. 42-5005) on top of this, or whether this fee is Mesa's own implementation of that charge — azdor.gov's Mesa city profile, which would clarify this, was blocked to automated access throughout this research.
No fee — the city's own fee schedule states directly: "Initial Certificate of Occupancy: No Charge." A duplicate copy costs $30 within 12 months of the original or $120 after that, and a temporary certificate is free for its first period but $500 (or $20/day if less) per 30-day extension in months 2 through 4, rising to $1,000 (or $40/day) per 30-day extension after that. A Change of Occupancy Permit, required when the use itself changes, runs a $500 minimum, priced like a renovation permit.
A further 4% Technology Improvement Fee applies, capped at $800 per transaction, plus a non-refundable permit deposit ranging from $170 to $33,000 by project size. Every commercial job must be performed by an Arizona-licensed contractor — Mesa issues no contractor license of its own and refers applicants directly to the state Registrar of Contractors.
Every figure here was read directly from the Fire Department's own published fee schedule. No separate line item for a food-truck fire inspection fee appears on this schedule, even though the city's own Mobile Food Vendor License page requires a fire inspection dated within the last 12 months as a submission document — that specific fee could not be confirmed and is not asserted here.
Arizona has no sales tax; it taxes the privilege of doing business, and the liability sits on the seller rather than the buyer. This licence is that tax's registration, which is why it is so often mistaken for a general business licence — but it is required only of people in taxable classifications, and it certifies nothing about fitness to trade. The state's own $12 is charged per location, and the department collects the city's fee on the same application and holds it in trust. There is no state renewal fee at all; eighty of the ninety-one cities charge nothing to renew either, and the eleven that do run from $2 in Chandler to $50 in Phoenix. Late renewal costs half the city fee plus a civil penalty. A use tax licence is free, and a trader under nineteen with less than $10,000 of gross proceeds needs no licence at all.
The published fee is $60 while the fee section says $50, and the difference is not an error in either: a separate subsection adds a $10 data-processing charge on articles of incorporation, and $25 on a foreign corporation's application for authority. So a for-profit corporation pays $60, a nonprofit $40 and a foreign corporation $175, and every published figure reconciles exactly. LLCs have no equivalent surcharge, which is the entire reason a corporation costs ten dollars more here. A certificate of disclosure must accompany the articles but carries no fee. Unlike an LLC, a corporation then files an annual report every year.
Only corporations file this — the Commission says so in terms, and the statute bears it out. The due date is not a common calendar day but a date the Commission assigns inside the anniversary month, so it differs for every corporation, and a six-month extension may be requested but only on or before that date. The penalty is unusually harsh: twenty per cent of the fee per month or part of a month, which is nine dollars a month on a $45 report and compounds until the corporation is administratively dissolved. Nonprofits are not penalised at all. Reinstatement is possible for six years at $100.
$50 to form, and then nothing — Arizona is one of very few states where an LLC files no annual report at all and pays no franchise tax. That is not an inference from a missing page: the phrase annual report appears exactly once in the whole of title 29 and it is in the limited liability partnership article, while the LLC Act's own fee schedule has no such line. Note that Arizona LLPs do file one, so the exemption is not general to unincorporated entities. Accelerated service is available on top of the $85 expedited fee, at $100 for next day up to $400 for two hours. Every Commission fee is non-refundable.
No government fee — the Commission charges nothing to publish and nothing to file the optional affidavit; what you pay is whatever the newspaper charges, and no official source sets that. In Maricopa and Pima counties you pay nothing at all, because the Commission publishes the notice itself on its public notice database, those being the only two Arizona counties above the eight hundred thousand population threshold the statute uses. The trap is that the test differs by entity type: for an LLC it is the statutory agent's street address, and for a corporation it is the corporation's own known place of business. An LLC formed by an owner in a rural county but using a Phoenix agent is exempt; a corporation in the same position is not. Publication must happen within sixty days of approval and not before it.
Three bands — $330 up to ten children, $1,330 to fifty-nine and $2,575 above — and this replaced a very different structure in August 2025, when the department converted a three-yearly fee of $1,000, $4,000 or $7,800 into an annual one. Any source quoting those older figures is out of date. The licence itself never expires; what keeps it alive is paying the annual fee, and a half-payment plan is available with the balance due within a hundred and twenty days of the anniversary. Licensing starts at five children — four or fewer is below the threshold entirely, and an unsubsidised four-child home needs no state licence or certificate at all. A residential group home of five to ten children is certified by the same department at $330, and a subsidised family provider caring for four or fewer is certified free by the economic security department for three years.
