About each permit
Body Art Facility Permit
A salon offering body art, piercing, or tattoo services anywhere in Salt Lake County needs this permit from SLCoHD: $275.00 plus $20.00 per station for plan review and the initial permit, $255.00 plus $20.00 per station to renew, $155.00 for a follow-up, or $125.00 plus $20.00 per station for a temporary permit, per the fee schedule published September 1, 2026. Health Regulation #19 was adopted by the Salt Lake County Board of Health on September 2, 1982, and last revised October 3, 2019, under Utah Code §26A-1-114. Without a body-art component, a salon offering only hair, nail, or skin services needs no permit under this program. SLCoHD's coverage is county-wide — all 23 incorporated municipalities plus the unincorporated area.
Food Establishment Permit
Any restaurant or coffee shop anywhere in Salt Lake County needs this annual permit from the Salt Lake County Health Department, priced from $275.00 to $770.00 by risk category (Risk 1 through Risk 4), per the SLCoHD fee schedule published September 1, 2026. SLCoHD's coverage is genuinely county-wide — all 23 incorporated municipalities plus the unincorporated area, no exceptions — under Utah Code §26A-1-103, the current statute requiring every Utah county to maintain a health department serving 'all incorporated and unincorporated areas in the county.' A business inside an incorporated municipality also needs that city's own separate business license; this SLCoHD permit does not substitute for it.
Mobile Food / Food Cart Permit
A food truck or food cart operating anywhere in Salt Lake County needs its own annual permit from SLCoHD: a Food Truck Permit is $350.00 at Tier 1 or $500.00 at Tier 2, and a Food Cart Permit is $230.00 at Risk 1, $275.00 at Risk 2, or $385.00 at Risk 3, per the SLCoHD fee schedule published September 1, 2026. SLCoHD's coverage is county-wide — all 23 incorporated municipalities plus the unincorporated area — so this permit reaches a unit based anywhere in the county. A one-off event instead uses the Temporary Food Establishment Permit listed separately here, and a separate mobile plan review, also on this fee schedule, is billed before a new unit's first permit.
HACCP Food Process Plan Review
A restaurant or food truck anywhere in Salt Lake County adding a specialized food process to its menu — uncooked or sushi preparation, sous vide, reduced-oxygen packaging, or a dog-friendly patio — needs a HACCP plan reviewed by SLCoHD before offering it, priced at a flat $640.00 on the fee schedule published September 1, 2026. This is separate from the food establishment's own annual permit and standard plan review listed elsewhere here. SLCoHD's coverage is county-wide — all 23 incorporated municipalities plus the unincorporated area.
Microenterprise Home Kitchen Permit
A home-based food business operating as a microenterprise home kitchen anywhere in Salt Lake County needs this permit from SLCoHD: $350.00 for a new permit, or $95.00 to renew, per the fee schedule published September 1, 2026. This is separate from, and an alternative to, the standard Food Establishment Permit listed elsewhere here, which applies to a commercial-location restaurant or coffee shop rather than a home kitchen. SLCoHD's coverage is county-wide — all 23 incorporated municipalities plus the unincorporated area.
Residential (Child)Care Food Service Inspection
A childcare facility that provides food service anywhere in Salt Lake County is inspected by SLCoHD for $115.00 flat, per the fee schedule published September 1, 2026, under Health Regulation #36. The childcare facility license itself is issued by the State of Utah Department of Health and Human Services, Office of Licensing; SLCoHD's role for a childcare business is limited to this food-service inspection. SLCoHD's coverage is county-wide — all 23 incorporated municipalities plus the unincorporated area.
Cosmetology Facility Permit
A salon offering cosmetology services anywhere in Salt Lake County needs this permit from SLCoHD: $305.00 plus $20.00 per station for plan review and the initial permit, $220.00 plus $20.00 per station to renew, $135.00 for a follow-up, or $110.00 plus $20.00 per station for a temporary permit, per the fee schedule published September 1, 2026. Health Regulation #20 was adopted by the Salt Lake County Board of Health on September 2, 1982, and last revised October 3, 2019, under Utah Code §26A-1-114. SLCoHD's coverage is county-wide — all 23 incorporated municipalities plus the unincorporated area.
Tanning Facility Permit
A salon offering tanning services anywhere in Salt Lake County needs this permit from SLCoHD: $190.00 plus $20.00 per station for plan review and the initial permit, $150.00 plus $20.00 per station to renew, $135.00 for a follow-up, and $40.00 for an operator certificate, per the fee schedule published September 1, 2026, under Health Regulation #25. Without tanning equipment, a salon needs no permit under this program. SLCoHD's coverage is county-wide — all 23 incorporated municipalities plus the unincorporated area.
Pool/Spa Operational Permit
A fitness studio operating a pool, spa, or hydrotherapy pool anywhere in Salt Lake County needs this permit from SLCoHD: $515.00 seasonal, $950.00 year-round, or $850.00 for a hydrotherapy pool, with plan review priced by square footage at $1,315.00, $1,830.00, or $2,350.00, a $195.00 follow-up, and a $30.00 Registered Pool Operator permit, per the fee schedule published September 1, 2026, under Health Regulation #02. Without a pool or spa, a fitness studio needs no permit under this program. SLCoHD's coverage is county-wide — all 23 incorporated municipalities plus the unincorporated area.
Vehicle Emissions I/M Station Permit
An auto-repair business operating a vehicle-emissions inspection/maintenance station anywhere in Salt Lake County needs this SLCoHD permit, priced at $190.00 for the station permit plus separate fees for a metal sign ($60.00), plan review ($50.00), each I/M test ($25.00), an inspector certificate ($20.00), the inspector certification exam and course ($155.00), inspector training ($20.00 per hour), and an inspector transfer or multi-station fee ($10.00), all per the fee schedule published September 1, 2026, under Health Regulation #22. SLCoHD's Regulations index page shows Regulation #22 currently has a proposed amendment, with a public-comment period ending Thursday, October 29, 2026, and a public hearing the same day; the fees above are from the currently adopted regulation and fee schedule and remain in force as of this record's verified date. SLCoHD's coverage is county-wide — all 23 incorporated municipalities plus the unincorporated area.
Temporary Food Establishment Permit
A restaurant or food truck serving food at a fair, festival, or other one-off event anywhere in Salt Lake County needs this permit rather than the annual Food Establishment or Mobile Food permits listed elsewhere here. The SLCoHD fee schedule, published September 1, 2026, prices it by risk tier and duration: Low Risk runs $70.00 for one day up to $330.00 for 14 days or a season, and High Risk runs $115.00 up to $455.00 across the same durations, with a $50.00 late fee for a late application or notification under either tier. SLCoHD's coverage is county-wide — all 23 incorporated municipalities plus the unincorporated area.
Massage Therapy Facility Permit
A salon or fitness studio offering massage therapy services anywhere in Salt Lake County needs this permit from SLCoHD: $180.00 for plan review and the initial permit, $30.00 to renew, $155.00 for a contracting business's initial permit, $135.00 for a follow-up, or $110.00 for a temporary permit, per the fee schedule published September 1, 2026, under Health Regulation #18. Without a massage-therapy service, a salon or fitness studio needs no permit under this program. SLCoHD's coverage is county-wide — all 23 incorporated municipalities plus the unincorporated area.
