About each permit
Food Handler Card
A food handler card is required, and it must come from a provider on the state's own approved list — the agriculture department publishes that list directly and states plainly that a card from an unapproved source will not be accepted by a local health department. Utah Administrative Code R392-103 sets the food handler permit fee at no more than $15, uniform statewide, payable to the local health department, and states that food handler permits are valid for 3 years statewide; a training provider may charge its own reasonable training fee on top. Confirm the provider's training price before budgeting for it.
Food Service Establishment Permit (local, not state)
No state fee, because a food establishment engaged primarily in service for immediate consumption is placed by the state agriculture department's own published guidance under the jurisdiction of the local health department where it operates, not under the state's own retail food registration. Utah has thirteen local health department jurisdictions, each setting its own fee under a shared statewide minimum sanitation standard. Confirm the figure with the specific local health department rather than assuming a statewide number, because none exists.
Retail Food Establishment Registration (grocery, processor or warehouse)
This is the state's own registration, and it applies only to a grocery, a food processor or a warehouse — never to a restaurant. The fee runs in five bands by size and function, from $100 for a cottage operation up to $1,250 for a large processor or grocer. Anything serving food for immediate consumption is licensed instead by a local health department under a different fee entirely — see the food service permit record.
