About each permit
Food Service Operation / Retail Food Establishment License
A restaurant, cafe, grocery or other food business in Warren County needs this license from the Warren County Health District, and the figure depends on whether the operation is commercial or non-commercial, its floor area and the risk level the district assigns. The amounts are the 'Total Fee' figures as published on the Warren County Health District's Environmental Health Fee Schedules page; the page prints one total for each line and no state/local split. Under ORC 3717.45(C)(1) and 3717.25(C)(1) the licensor collects the amount the state director of health sets 'at the same time the fee is collected.' The page states that 'A late fee of 25% of the local fee is assessed on facilities that do not submit application and payment postmarked by March 1st.' The district's renewals page states that license renewals for food facilities are completed through the Accela Citizen Access Portal, with payment online, by phone at 513-695-1220 or in person, and a $3.00 fee for credit card payments. The page itself states: 'Fees are subject to change at any time. We will do our best to keep these fees updated. Please verify the fee when calling in or during your visit.' The Health District's history page states that 'Warren County's health department has evolved into the Warren County Combined Health District, signifying not only health services for the county, but also for the cities of Franklin, Lebanon, Mason, and Springboro.'
Food Facility Plan Review (New FSO/RFE)
Before a new food service operation or retail food establishment is built in Warren County, its plans go to the Warren County Health District for review. The fees page lists 'New Commercial or Non-Commercial Facility Plan Review $300.00', the district's Accela page states that 'The plan review fee is $300.00', and its plan review packet states that 'A plan review fee of $300.00 must be submitted along with plans.' The packet adds that 'A plumbing permit cannot be obtained until plan approval through our office is complete,' that a letter on the status of the plans will be sent within thirty days together with a license application, and that the district must be contacted for a licensing inspection before the operation opens. Plans and applications are submitted through the Accela Citizen Access Portal. The Health District's history page states that 'Warren County's health department has evolved into the Warren County Combined Health District, signifying not only health services for the county, but also for the cities of Franklin, Lebanon, Mason, and Springboro.'
Mobile Food Service Operation / Retail Food Establishment License
A food truck, trailer or coffee cart licensed through the Warren County Health District pays by risk level: $178.00 for a high-risk mobile operation and $103.00 for a low-risk one, as published on the Warren County Health District's Environmental Health Fee Schedules page, which also states 'There is no fee for Mobile Plan Review.' The district's mobile plan review guidelines state that 'Licensed unit can be used anywhere in the state of Ohio.' Its Accela mobile page lists the documents to submit, including the menu, equipment list, commissary or shared kitchen agreement and person-in-charge certification in food safety for high-risk mobiles. The fees page states that 'Seasonal and mobile facilities must make payment prior to the start of their operational period.' The page itself states: 'Fees are subject to change at any time.
Temporary Food Service Operation / Retail Food Establishment License
A business serving or selling food at a festival, fair or other short event in Warren County needs a temporary license from the Warren County Health District. The district's temporary packet (footer 'Temporary Plan Review Application 10/26') defines a temporary operation as one that prepares or serves food 'for a period of no longer than 5 consecutive days,' states 'The fee for a Temporary FSO/RFE license is $100.00. A noncommercial license fee is $50.00' for government agencies, churches, schools, youth groups and 501(c)(3) organizations, and requires the questionnaire, application and license fee 'at least 5 business days prior to the event.' The packet lists sales that need no temporary license, such as beverages that are not time/temperature controlled for safety, prepackaged non-TCS foods with less than 200 cubic feet of display, and uncut fresh fruits and vegetables. Payment can be mailed to 416 S. East Street, Lebanon, or made by card at 513-695-1220. The Health District's history page states that 'Warren County's health department has evolved into the Warren County Combined Health District, signifying not only health services for the county, but also for the cities of Franklin, Lebanon, Mason, and Springboro.'
Public Swimming Pool, Spa and Special Use Pool License
A gym, studio, club or childcare center in Warren County that runs a public swimming pool, spa or special use pool needs a license for each one from the Warren County Health District; a business without a pool or spa has no pool license to buy. The figures are as published on the Warren County Health District's Environmental Health Fee Schedules page. ORC 3749.04(F) provides that 'An annual license fee established under division (D) of this section shall include any additional amount determined by rule of the director of health,' so the published amount includes that state amount. ORC 3749.04(B) requires an existing pool to apply during April, a new pool to apply at least thirty days before opening, and (C) makes each license 'effective from the date of issuance until the last day of May of the following year.' The district's pool page states that its staff inspect all public swimming pools one to two times a month and that new or substantially altered pools need plan approval from the Ohio Department of Health before work starts. The Health District's history page states that 'Warren County's health department has evolved into the Warren County Combined Health District, signifying not only health services for the county, but also for the cities of Franklin, Lebanon, Mason, and Springboro.'
Body Art Establishment License
A salon or studio in Warren County that offers tattooing, body piercing or permanent makeup needs a body art establishment license from the Warren County Health District; a salon without those services has no body art license to buy. The figures are as published on the Warren County Health District's Environmental Health Fee Schedules page. The district's body art page states that body art is regulated under ORC Chapter 3730 and OAC Chapter 3701-9, that staff conduct 'at least one inspection per year at each facility,' and that 'In order to open up a tattoo, body piercing, or permanent make-up facility, approval must first be obtained from local zoning, and facility plans must be submitted to the Health District and the Warren County Building Department.' The district's renewals page lists body art among the licenses renewed through the Accela Citizen Access Portal. The Health District's history page states that 'Warren County's health department has evolved into the Warren County Combined Health District, signifying not only health services for the county, but also for the cities of Franklin, Lebanon, Mason, and Springboro.'
Plumbing Permit, Commercial Plumbing Plan Review and Plumbing Registration
Plumbing work in a new building, a build-out or a remodel goes through the Plumbing Division of the Warren County Health District, whose page states that its State of Ohio Certified Plumbing Inspectors are 'responsible for plan review and inspection of all residential commercial plumbing work whether it is a remodel or new construction in Warren County,' with final approval of the plumbing before occupancy. The figures are as published on the Warren County Health District's Environmental Health Fee Schedules page, which states that a reinspection fee applies to each inspection after the initial six, allowing two inspections for each stage of the permit (first rough, second rough and final), and that 'Plumbing permit fees are doubled if any work has been completed prior to submitting permit application.' The same page lists a Contractor's Registration Fee of $100.00 and an Installer's Registration Fee of $50.00. For a new food facility, the district's food plan review packet states that a plumbing permit cannot be obtained until the food plans are approved. The page itself states: 'Fees are subject to change at any time. We will do our best to keep these fees updated. Please verify the fee when calling in or during your visit.'
Small Flow On-Site Sewage Treatment System (SFOSTS) Permit
A restaurant, childcare center, clinic or garage on a site that treats its own sewage, rather than connecting to a sanitary sewer, uses a small flow on-site sewage treatment system permitted by the Warren County Health District. ORC 3718.01(R) defines that system as 'a system, other than a household sewage treatment system, that treats not more than one thousand gallons of sewage per day' (and that needs no NPDES or injection well permit), which is what makes it the business category. The figures are as published on the Warren County Health District's Environmental Health Fee Schedules page. The district's sewage page states that it issues installation permits and inspects all new, replacement and altered sewage treatment systems in Warren County, that a site review application follows the soil evaluation and system design, that 'A Sewage System Permit to Install and operating permit must be obtained from the WCHD prior to installation,' and that the finished system is placed in the Operation/Maintenance Program. The Health District's history page states that 'Warren County's health department has evolved into the Warren County Combined Health District, signifying not only health services for the county, but also for the cities of Franklin, Lebanon, Mason, and Springboro.'
