About each permit

Food Establishment License

A restaurant under 25,000 square feet runs $359 at the lowest risk level to $942 at the highest; the same tiers roughly double above 25,000 square feet, reaching $1,946. Unpaid past the March 1 due date draws a 25% surcharge. Applications go through the ezTrak system, and no equipment may be purchased before the plan is approved.

Food Safety Training Certification

These are state requirements delivered and enforced locally through the Cincinnati Health Department, not a purely city-invented rule. The manager certification applies specifically to the two highest risk levels; lower-risk establishments only need the shorter, cheaper Person in Charge course.
