About each permit
Child Care Centre Sanitation Plan Review and Approval
15A NCAC 18A .2802(a) provides that construction plans drawn to scale and specifications for a new child care center that is not a chain or franchise child care center shall be submitted by the operator or the operator's designee to the local health department that serves the county in which the child care center is located for review and approval prior to initiating construction — in Forsyth County, the Division of Environmental Health. The same rule continues, and this part is easy to miss: plans drawn to scale and specifications for changes to building dimensions, kitchen specifications, or other modifications to existing child care centers, including chain or franchise child care centers, shall also be submitted to the local health department for review and approval prior to initiating construction. So an established franchise centre reworking a kitchen goes to the county even though its original prototype plans went to the State; only prototype chain or franchise plans are reviewed in Raleigh. The rule's authority is G.S. 110-91 and G.S. 110-92, and Forsyth County Environmental Health lists day cares among the establishments it inspects. No fee for this review is published on the county's FY 26-27 Fee Schedule, whose Environmental Health block carries no child care line, so the amount should be confirmed with Environmental Health on (336) 703-3225 before applying. This county sanitation role sits alongside, not instead of, the operating license a centre needs from the NC DHHS Division of Child Development and Early Education, and a centre with a kitchen also meets the food establishment records on this page while one on a non-sewered site meets the septic record.
Mobile Food Unit Plan Review
A food truck or trailer operating in Forsyth County has its plans reviewed by county Environmental Health at $170.00 per unit, printed as MFU Plan Review on the Forsyth County Public Health FY 26-27 Fee Schedule. The review is where the commissary question is settled, and the statute is specific about it: G.S. 130A-248(c1) provides that a mobile food unit shall meet all of the sanitation requirements of a permitted commissary or shall have a permitted restaurant or commissary that serves as its base of operation, so an operator without a base kitchen has to solve that before the unit can be permitted. The plan review is a one-off charge on the unit; the recurring cost is the $120 annual food establishment permit fee that G.S. 130A-248(d) assigns to NC DHHS, listed separately, and the two should not be added together as a single county figure. A truck that also trades at fairs and festivals may separately need the $75.00 temporary food establishment permit for those events, also recorded separately. Forsyth County Environmental Health runs this programme countywide, in Winston-Salem and every other municipality in the county, since it acts for NC DHHS under G.S. 130A-4(b) rather than under a municipal delegation. Plans and payment go to 799 N. Highland Avenue, Winston-Salem, (336) 703-3225. The figure is from the county's current FY 26-27 schedule, which runs with the county fiscal year from July 1, 2026 to June 30, 2027.
Food Service Plan Review
A restaurant or coffee shop building out a kitchen in Forsyth County submits plans to the Division of Environmental Health and pays $250.00, as published in the Forsyth County Public Health FY 26-27 Fee Schedule, an eleven-page document the county's own Public Health Publications page carries as the FY26-27 Fee Schedule under Forsyth County HHS branding. That amount is exactly the statutory ceiling: G.S. 130A-248(f) allows a local health department to charge a fee not to exceed two hundred fifty dollars for plan review by that local health department, so the county is charging the maximum the General Assembly permits rather than a locally invented figure. Chains and franchises are treated differently, and the saving is worth knowing about: G.S. 130A-248(e1) provides that plans for a franchised or chain food establishment that have been reviewed and approved by the Department shall not require further review and approval by any local health department, and the prototype review the State itself performs under subsection (e) carries its own $250.00 fee with the statute adding that no food establishment paying a fee under subsection (e) is liable for a fee under subsection (f). A single-site independent operator therefore pays the county; a franchisee opening to an approved prototype pays the State once instead. This charge is separate from the $120 annual permit fee that NC DHHS charges under G.S. 130A-248(d), listed separately, and from any building, trade or zoning permit the joint Inspections Division charges for the same fit-out. The county's fiscal year runs July 1 to June 30, so the FY 26-27 schedule governs the amount for work reviewed between July 1, 2026 and June 30, 2027.
Foodservice Remodel Plan Review
Forsyth County prices a remodel of an existing food service operation lower than a new build: the Foodservice Remodel plan review line is $140.00 on the Forsyth County Public Health FY 26-27 Fee Schedule, against $250.00 for a new Food Service Plan Review. The practical trigger is any change to the kitchen that needs plans — moving or adding equipment, reworking a dish area, changing the layout of a prep line — in a space that already holds a food service permit, as distinct from fitting out a space that has never been permitted. Both figures sit inside the ceiling that G.S. 130A-248(f) sets for a local health department's plan review fee. The amount is charged by Forsyth County Environmental Health and is separate from the $120 annual permit fee that NC DHHS charges under G.S. 130A-248(d), and separate again from the building, mechanical, plumbing or electrical permits the joint Winston-Salem/Forsyth County Inspections Division charges for the same work. A remodel that changes the ownership of the business rather than the kitchen is a different step: G.S. 130A-248(c) requires the new owner or lessee to apply for a new permit, with a transitional permit available. Plans and payment go to the Division of Environmental Health at 799 N. Highland Avenue, Winston-Salem, (336) 703-3225. The schedule is the county's FY 26-27 document, running with the county fiscal year from July 1, 2026 to June 30, 2027.
Push Cart Plan Review
A push cart is priced separately from a full mobile food unit in Forsyth County: the FY 26-27 Fee Schedule prints PushCart Plan review at $113.00, against $170.00 for a mobile food unit. The base-of-operations rule is stricter for a cart than for a truck, and it comes straight from the statute: G.S. 130A-248(c1) provides that a permitted restaurant or commissary shall serve as a base of operations for a pushcart, with no option of the cart itself meeting commissary requirements. This is the record that fits a coffee or espresso cart, a hot dog cart or a small beverage cart as much as a food cart. The plan review is one-off; the recurring charge is the $120 annual food establishment permit fee that NC DHHS charges under G.S. 130A-248(d), listed separately. A cart trading only at short-term events may instead be permitted as a temporary food establishment at $75.00, also recorded separately, and the county's own temporary food establishment page notes that vendors selling only certain items, among them popcorn, candied apples, cotton candy, funnel cakes, doughnuts, peanuts and commercially prepackaged chips and candy, do not need an operational permit from the Department though they should contact the NC Department of Agriculture. Plans and payment go to Environmental Health at 799 N. Highland Avenue, Winston-Salem, (336) 703-3225.
Temporary Food Establishment (TFE) Permit
Forsyth County's own page states that a Temporary Food Establishment permit is required to sell food or drink at a special event and that the permit is issued in conjunction with a fair, carnival, circus, public exhibition or other similar gathering that operates for a period of 21 days or less. The amount is set by statute rather than by the county: G.S. 130A-248(d2) provides that a local health department shall charge each temporary food establishment and each limited food services establishment a fee of seventy-five dollars ($75.00) for each permit issued, and the county's FY 26-27 Fee Schedule prints the matching $75.00 under TFE Temporary Food Establishment Fee. Temporary food establishments are expressly excepted from the $120 annual permit fee by G.S. 130A-248(d), so a vendor at a single festival does not owe both. The submission rules are unusually strict and the county states them plainly: applications and payments for all TFE applications are only accepted via postal mail or in person at the Environmental Health Division, 799 N. Highland Avenue, with no over-the-phone, faxed, emailed or online payments accepted, and payment must be included with the application at the same time. The same page adds that vendors who sell only dipped ice cream, shaved ice, popcorn, candied apples, cotton candy, funnel cakes, doughnuts, peanuts and commercially prepackaged chips and candy do not have to get an operational permit from the Department, though they should contact the NC Department of Agriculture on 919-733-7366 to determine whether a permit is needed from that agency — a carve-out that takes a large share of festival snack stalls out of this requirement. The programme is countywide, so the same $75.00 applies at an event inside Winston-Salem or any other municipality in the county.
Limited Food Service Establishment Permit
A limited food service establishment permit is a narrow category and an ordinary commercial restaurant or coffee shop cannot hold one. G.S. 130A-248(a4) provides that limited food service establishment permits shall be issued only to political subdivisions of the State, establishments operated by volunteers that prepare or serve food in conjunction with amateur athletic events, or establishments operated by organizations exempt from federal income tax under section 501(c)(3) or section 501(c)(4) of the Internal Revenue Code. It is recorded here because a nonprofit or municipal operator running a kitchen, concession or cafe in Forsyth County reaches this permit rather than a full food establishment permit, and because the fee is the same $75.00 that G.S. 130A-248(d2) fixes for a temporary food establishment: the statute directs a local health department to charge each temporary food establishment and each limited food services establishment a fee of seventy-five dollars for each permit issued, and Forsyth County's FY 26-27 Fee Schedule prints Limited Food Service Permit Fee at $75.00. Limited food service establishments are excepted from the $120 annual permit fee under G.S. 130A-248(d), so the $75.00 is the whole permit charge. A qualifying operator that also builds or remodels a kitchen still meets the county's plan review fees listed separately. Applications go to Forsyth County Environmental Health, 799 N. Highland Avenue, Winston-Salem, (336) 703-3225.
On-site Wastewater (Septic) Evaluation and Construction Authorisation
Forsyth County Environmental Health enforces on-site sewage disposal laws and regulations, performs soil and site evaluations to determine whether property not served by a public sewer system is suitable for a septic tank system, and inspects the installation of septic tank systems. The line a commercial business pays is SS1, whose own label on the county's FY 26-27 Fee Schedule reads '480-1500 gpd (business or church)' at $489.00 — not the SSA residential soil site application at $232.00 per lot, which prices a three to six bedroom house. Higher design flows move up the schedule: $739.00 for 1,500 to 3,000 gallons a day and $2,604.00 above 3,000 gallons a day, which is where a large restaurant, a sizeable childcare centre or a veterinary hospital on a rural site can land. The evaluation and the construction authorisation are separate charges, so a new conventional gravity system on a commercial site is $489.00 plus $265.00 before any revisit, and a pump installation adds $71.00. Taking over a property with an existing system is cheaper but not free: a T&J panel repair is $232.00, a redraw of the improvement permit or construction authorisation is $43.00 and a Health Department Release is $64.00. Every revisit is $64.00. The programme is countywide rather than confined to the unincorporated area, because Environmental Health acts for NC DHHS under G.S. 130A-4(b) across the whole county. A business on a public sewer has no county septic step at all. Applications go to Environmental Health at 799 N. Highland Avenue, Winston-Salem, (336) 703-3225, and the figures are from the county's FY 26-27 schedule running July 1, 2026 to June 30, 2027.
Tattoo Artist Annual Permit (applied for and paid through Forsyth County)
A salon in Forsyth County where tattooing or permanent makeup is performed meets this permit, and a salon that offers neither does not — the scope consequence is the whole point of the record, since hair, nail and skin work is licensed by state boards rather than by the county. The permit itself is a state permit administered locally: G.S. 130A-283(b) provides that no person shall engage in tattooing without first obtaining a tattooing permit from the Department, and (c) has the Department, acting through the local health department, inspect the premises, instruments, utensils, equipment and procedures of the applicant, with the permit valid for one year and renewable annually by applying to the Department. Forsyth County Environmental Health is the local health department for that purpose under G.S. 130A-4(b), lists tattoo parlors among the establishments it inspects, and prices the permit on its own FY 26-27 Fee Schedule at $140.00 under code TAP as the Tattoo Artist Annual Permit Fee. The permit attaches to the artist rather than to the shop, so a studio with several artists pays per artist and an artist joining a different establishment applies again. Forsyth County also lists tattoo educational workshop charges on the same schedule, but the schedule states two different sets of amounts for them on one page, so no workshop figure is published here; the $140.00 artist permit is unaffected by that and is the figure a salon needs. Applications and payment go to Environmental Health, 799 N. Highland Avenue, Winston-Salem, (336) 703-3225.
Seasonal Public Swimming Pool Annual Operation Permit
This record covers a SEASONAL pool only, and the distinction decides the price. G.S. 130A-281 provides that no public swimming pool may be opened for use unless the owner or operator has obtained an operation permit issued by the Department under rules adopted pursuant to G.S. 130A-282, and G.S. 130A-280(b)(2) defines a public swimming pool to include municipal, school, hotel, motel, apartment, boarding house, athletic club or other membership facility pools and spas — the 'athletic club, or other membership facility' wording is what brings a gym's pool or spa into scope. Forsyth County Environmental Health permits and inspects all public swimming pools in the county, approximately 350 of them on its own count. The fee line on the county's FY 26-27 Fee Schedule is 'SAF Seasonal Swimming Pool/Aquatic Center Annual Fee (Per Permit)' at $148.00, reduced to $74.00 where the required application, data sheet and fee are paid prior to the second Friday of March — an early-payment discount worth half the fee. Forsyth County's own Application for Public Swimming Pool Operation Permit draws the line between the two classes: 'NOTE: Pools opening prior to April 1 or remaining open after October 31 are considered year round pools.' A year-round pool is therefore a separate class, the FY 26-27 schedule prices only the seasonal permit, and no annual amount for a year-round pool is published in it — so a fitness studio with an indoor pool or spa open through the winter, which is the usual case for a gym, should contact Environmental Health on (336) 703-3225 for the amount charged rather than budget the seasonal figure. The inspection burden differs too: 15A NCAC 18A .2511(a) provides that pools which open on or after April 1 and close on or before October 31 shall be inspected at least once during the period of operation, while all other pools shall be inspected at least twice a year. A fitness studio without a pool or spa on site has no pool permit to obtain. The programme is countywide, inside Winston-Salem and every other municipality.
Swimming Pool / Aquatic Center Plan Review and Revisit
Building a pool, spa or aquatic centre for a fitness studio in Forsyth County, or rebuilding an existing one, means a plan review by county Environmental Health at $269.00, printed under code SPR on the Forsyth County Public Health FY 26-27 Fee Schedule, plus $64.00 under code SPRF for each revisit the project needs. The revisit line is the one that catches operators out: it is charged per return visit rather than once, so a pool that is not ready at the first inspection accumulates the charge. Plan review is a one-off cost and is separate from the recurring seasonal operation permit at $148.00, or $74.00 with early payment, listed separately, and a new pool pays both in its first year. The technical standards are 15A NCAC 18A .2500, and the permit the review leads to is the operation permit that G.S. 130A-281 requires before a public swimming pool may be opened for use. The programme is countywide — Forsyth County Environmental Health permits and inspects all public swimming pools in the county — so a pool inside Winston-Salem is reviewed by the county rather than by the city. A pool's electrical equipment is permitted separately by the joint Winston-Salem/Forsyth County Inspections Division at $138.00, listed separately, and a pool on a site without public sewer or public water also meets the county's septic and well records. A fitness studio without a pool or spa has no plan review to submit. Plans and payment go to 799 N. Highland Avenue, Winston-Salem, (336) 703-3225.
Private Water Supply Well Construction, Abandonment and Sampling
Forsyth County Environmental Health enforces state and local private drinking water well laws and rules and handles the permitting and inspection of new wells, well repairs and well abandonment. A business on a rural site in the county — an old farm building becoming a veterinary clinic, a salon, a small restaurant or a childcare centre — meets this record rather than a public water connection. Two things are worth reading carefully on the county's FY 26-27 Fee Schedule. First, the well construction permit line appears twice, under the identical code and description 'WCP Water Supply Well Const. NFHC Permit', once at $398.00 and once at $489.00, with no printed criterion distinguishing the two amounts, so the safe course is to ask Environmental Health on (336) 703-3225 which applies before committing to a figure. Second, water sampling is priced per analyte rather than as a single package, so the total depends on what the site needs: bacteria alone is $52.00, nitrate/nitrite and fluoride are $54.00 each, inorganic is $102.00 and rises to $108.00 with nitrate, and pesticide, petroleum and organic (VOA) panels are $121.00 each, with a uranium plus three metals panel at $103.00 and iron and sulfate reducing bacteria at $86.00 and $96.00. Abandoning a well properly is a permitted act in its own right at $176.00, which matters when a business takes on a property with a disused or failed well. A site on a public water supply has no county well step. The figures are from the county's current FY 26-27 schedule, running July 1, 2026 to June 30, 2027; applications go to 799 N. Highland Avenue, Winston-Salem.
